Medical Form Procedures & Hints
All Scouts attending summer camp MUST have a completed Class 3 medical form.
This includes adults who will be spending more than 48 hours in camp OR who want to swim or boat.

Form Process
1. Download the form and fill out Sections I, II. III, IV, and VI as much as possible.
2. Schedule the medical checkup with your physician.
The scout and adult must have a physical within one year prior to the last day of camp.
For Floodwood, the physical exam must have been AFTER July 19, 2007.
For Turrell it must be AFTER August 2, 2007.
ABSOLUTELY NO EXCEPTIONS! DON’T

3. If you child has had a physical since the cutoff date above, give the form to the Dr’s office to fill out and return.
4. Once you get the form back from the Doctor, review it for completeness.

6. All the medical forms will be reviewed by Scout Leaders
before summer camp and parents will be given a chance to correct problems. It

7. On the first day of summer camp, the medical staff will review each medical form.
If errors exist, you will have 24 hours to fix the errors with your doctor. After 24 hours, your Scout must be removed from camp.
8. At the end of camp, the forms will be returned to the troop.
Form Instructions & Hints
Fill out the form completely leaving no empty spaces. If not applicable, write “N/A”.
Section II Emergency Medical Information – If non apply, write “none”
Section III Parental Statement – Applicant
Signature , the scout MUST sign it!
Section IV Immunizations - You can attach a copy of the
immunization record from the Dr’s office.
Section V Evaluation – if there are no restrictions write,
“Healthy child, No
restrictions”
The Doctors
office stamp is equal to a signature.
Section VI Medical History – Date of most recent complete physical (see item #2 above).
Medications – You can attach a separate sheet with
medications and dosage. Scouts will be
required to have their medications in a plastic baggie with their name and
Troop 49 Site M on it and turn it in to the
Section VII Health Exam – Check box if NORMAL. If no problems exist, write in comments,
“Normal Child”
