BSA Annual Health and Medical Record
Each scout and leader is required to have this form on file (with a doctor's or other licenced practitioner's signature) for attending summer camp and any outing longer than 72 hours in length. The signed form is valid for one year from the date signed. -

This document is published by the National BSA and required reading for all adults who will participate in delivering the scouting program to our youth. -
Tour Plan Form

Lenapehoking Lodge IX
The Order of the Arrow is Scouting's National Honor Society. For more than 90 years, the Order of the Arrow (OA) has recognized Scouts and Scouters who best exemplify the Scout Oath and Law in their daily lives. This recognition provides encouragement for others to live these ideals as well. This site is for the local Lodge which the member scouts of our troop belong to.
